Causal Diagrams

Use the application’s interactive and transactional diagram functionality to build the potential causal path of any risk, whether operational / disaster / functional / strategic etc.

Incident Investigations

The interactive tool set for the investigation analysis of incidents and accidents allows the user to build a transactional (simply drag the selected cause to the diagram) causal path – the outputs automatically generate a complete investigation report with the full analysis of all causes and the MHSA 11.5 Report. A suite of dashboards enable the manager to quickly drill into his area of responsibility and within seconds view the overall causes of incidents.  The high configurability of IRMA allows the organization to design their own investigation methodology i.e. ICAM / Causal Analysis / RCAT / ICAT / Fault Tree / Bowtie with any of these methodologies – the adjacent diagram (top) is a Bow Tie combined with a Root Cause Analysis Technique.


The inspection module allows the organisation to design and compile all the relevant checklists and checking items for each working area / work area type / geographical areas - for all the different departments and functions performing inspections. For each checking item a risk value can be assigned that will represent the risk associated to the checking item giving the circumstances of the risk – the dashboards can promptly indicate to management the risk profile trend – the operation area of concern – the specific risk category - the risk is increasing – the risk profile can be drilled into the specific checking item causing the risk increase.
